Frequently Asked Questions - State ID
1. Where can I go to get a state ID card?
  Effective January 2, 2013, state ID cards will be issued at all County of Maui DMVL Offices.
     
2. What is the term for the state identification card?
  Residents are issued cards that expire on the eighth birthday after the year of issuance.
     
3. What are the fees for issuance of the state identification card?
  Initial and renewal - All ages - $40
     
4. What documentation am I required to provide?
  You are required to provide proof of legal name, date of birth, legal presence, social security number and principal residence (Two documents required).
     
5. Can I request to renew my state ID card without coming into your office?
  No.  All applicants must accomplish an in-person issuance.  Only renewing applicants 80 years old and over who have previously submitted their documentation and processed as an in-person issuance may opt to subsequently renew by mail.
     
6. Am I required to obtain a state ID card?
  No, it is a voluntary program.  The state ID card is a government issued identification and an alternative to the Hawaii driver license, for those who do not drive a vehicle.
     
7. How early can I renew my state identification card?
  The earliest you can renew your state identification card is six months before the expiration date.
     
8. What happens if I let my state identification card expire?
  You have 364 days after the expiration date to renew your state identification card.  If you do not renew your state identification card within the 364-day period, you will be processed as a new applicant.
     
9. How do I apply for a duplicate state identification card?
  You may apply for a duplicate state identification card at any County of Maui DMVL Office.  Complete a State of Hawaii Identification Application and indicate that you are requesting duplicate state identification card.
     
10. How do I renew my state identification card by mail?
  Renewal by mail requests are limited to applicants who are 80 years of age or older.
If you are 80 years of age or older you may renew by mail provided you are not temporarily authorized to be in the U.S.  
  Complete a State of Hawaii Identification Application
  Provide your signature on a separate 8 ½ x 11 sheet of paper.  This signature will be used as your signature on your card. .   If your image is found in our database, both the file photo and the signature will be utilized on the card.  Otherwise, your state identification card will indicate "valid-without-photo".
  Your renewal application packet must include original documents for proof of legal name, date of birth, social security number, legal presence and principal residence if you have not previously provided these to the DMV.   Notarized copies are not acceptable.

11. How do I get a duplicate state identification card if I am currently out of state?
  Complete DMVL419-12-12 - Duplicate SID Request form.
  i. Enter your name
  ii. Email Address
  iii. Contact Number
  iv. Select Reason for Duplicate
  Complete and sign the State of Hawaii Identification Application form.
  Enclose payment of $6.00 via certified check or money order payable to County of Maui.
