Instruction Permits
Class Three Instructional Permit
To apply for a class 3 Instruction Permit, the applicant must be at least fifteen (15) years and six (6) months of age. All applicants must present original or certified copies of the following: 

  1. Proof of Social Security Number. 
  2. Proof of Date of Birth. 
  3. If the applicant is under eighteen (18) years of age:
    1. Birth Certificate is required.
    2. An Affidavit must be submitted in connection with the desire of the aforesaid child to secure a license to drive. This affidavit must be witnessed by a person authorized to administer oaths and must signed as follows: 
      • If both the father and mother of the applicant have custody, both the father and mother must sign the affidavit. 
      • If only one parent has custody of the applicant, that custodial parent must sign. 
      • If neither parent has custody of the applicant and the applicant has a custodial guardian(s), all the custodial guardian(s) must sign. 
      • If neither parent has custody of the applicant and the applicant has no custodial guardian(s), a responsible person who is willing to assume the obligation may sign.
    3. The person(s) who signed shall be jointly and severally liable with the minor applicant for any damages caused by such negligence or misconduct. 
  4. Pass the required vision test. 
  5. Pass a 30 question multiple choice written examination. If you obtain more than 6 errors, you will fail the examination and will be required to wait seven days before you are eligible to retake the examination. The appropriate examination fee will be collected before the test is administered. Failure to pass the examination shall not be grounds for a refund.   6. Payment of the appropriate fees

Class 4 Instructional Permit

To apply for Class 4 instruction permit the applicant must meet the following requirements:
  • Be 21 years of age or older
  • Possess a valid class 3 state of Hawaii driver's license
  • Pass the required vision test
  • Submit a medical examiners certificate 
  • Submit a clearance letter from the Traffic Violations Bureau if the applicant has any outstanding citations. The Traffic Violations Bureau is located at 2145 Main Street in Wailuku. Telephone: (808)-244-2800
  • Payment of the appropriate fees

Proof of Social Security Number
The following are acceptable proof of social security number:
  • Original social security card
  • State of Hawaii Identification card
  • Medicaid card
  • Military identification card or DD-214

Proof of Name
The following are acceptable proof of name:
  • State certified birth certificate
  • State of Hawaii identification card
  • Certified divorce decree with legal name change for females to assume maiden name
  • U.S. Immigration and Naturalization documents
  • State certified marriage certificates
  • Certified decree of name change - State documents by courts or lieutenant governor’s office, State of Hawaii

Issuance of Permit
The instruction permit will be issued upon meeting vision requirements, successfully passing a general driving multiple choice test, and payment of the appropriate permit fees
